Although every effort is made to create a safe environment, I realize there is always a risk of accident. If necessary, I authorize Bonita’s School of Ballet to administer first aid treatment and/or emergency treatment for my child on my behalf. I further release Bonita’s School of Ballet and its employees from liabilities for injury or damages arising out of personal injury of any kind.
In teaching the art of dance, it is entirely appropriate for a teacher to put their hands on a student to correct the student's posture, the physical line, position of the student's body or part of his/her body, or to help a student hold a position. BSOB does not tolerate any teacher-student contact that is harmful or inappropriate. In enrolling their child in BSOB, parents acknowledge that it is BSOB practice to correct students with physical contact.
Bonita’s School of Ballet is granted permission to use dancer’s image in advertisements, news releases and on its website.
I understand that Bonita's School of Ballet is a private dance school and I certify that my dancer is not enrolled in any other dance classes other than an academic school of the arts.
Please read and check the following to signify that you understand studio policies. (The check will act as an electronic signature) There is a registration fee of $25 for new students and $20 for returning students due at the time of registration.
Tuition payments are due on or before the first day of each month. After the 10th of each month, a $10 late fee will be applied to all late payments.
All returned checks will incur a $35 fee.
Students with unpaid tuition may not be allowed to participate in class until fees are paid in full or arrangements have been made.
Missed classes & tuition payments are not refundable. Students should contact studio directly to inquire about attending another age appropriate make up class. Refer to studio policies for more details.
Tuition at Bonita's School of Ballet is a yearly course fee broken down into nine monthly payments for your convenience. Monthly tuition fee will remain the same regardless of weeks in the month, school vacation closings, or the number of absences in a month.
Each student who is performing in the recital will need to pay a $40 Recital Fee by April 1st.
Tickets to the recital will cost $5/person.
A costume deposit of $25 will be due at registration. A second recital costume installment of $25 must be paid by the first week in November. If dancer takes more than one class per week, the second installment should be $50. The balance is due in February. Costume payments are non-refundable.
